Acne inversa (Hidradenitis suppurativa) is a chronic skin disease with inflammation of the apocrine sweat glands. It mainly affects the skin of the axillary region and genitals. It is characterized by the appearance of red bumps or nodules which are often filled with purulent contents. In a large percentage of cases, drug therapy is unsatisfactory, so a gold standard in acne inversa therapy remains surgical treatment.
For the first time the Bulgarian Society of Dermatological Surgery (BULSDS) offers treatment of acne inversa / hidradenitis suppurativa, in the form of various invasive methods:
1. Serial surgical excisions under local anesthesia.
2. Complete removal of one or two regions (axillary or inguinal), within one surgical intervention, under general or spinal narcosis with:
a) subsequent secondary healing of the surgical defect or
b) application of surgical flaps in order to maximally adapt wound edges and avoid massive scarring in secondary healing
The Bulgarian Society of Dermatological Surgery (BULSDS) introduces the determination of a first draining/ sentinel lymph node by radioactive labeling with technetium, followed by SPECT-CT in order to maximize its exact localization and visualization.
At the same time, a removal of a sentinel lymph node is performed in combination with its intraoperative express histological examination – express frozen histological sections (Gefrier).
